Asthma Survey: Data Collection Guidelines

Conducting the Survey

Ask permission from teachers to give the asthma survey in their classes and arrange a good date and time to give the survey in classes.

Download the Survey Introduction which you will read to the participating students.

Download the passive Parental Permission Form.

Download the Survey Form which each student will fill out.

Make as many copies of the survey and parental permission form as are needed to collect data from students in the classes identified

In each class:

1. The day before you plan to give the survey, distribute the parental permission form to students in the participating classes
2. When you give the survey in a class:
• Read the survey introduction
• Distribute the survey to students only
• Collect the completed surveys and return them to your team's faculty advisor

Entering your Data

Assign each survey a numerical code. For example, "W02001" would mean W = Winter, 02 = 2002, and 001 = survey number 1.

Download the Excel Spreadsheets to your computer and enter the survey data.

When entering the data, use the appropriate Response Codes for each field in the spreadsheet.

Periodically check the accuracy of the data entered.
